This form serves as an agreement between Custom Creations by Jackie and the customer for the purchase of products and services. The customer agrees to the terms and conditions outlined below.

- Custom and clearance items are non-refundable.

- Customer information is collected solely for transaction purposes and will not be shared with third parties without consent.- Customers are responsible for providing accurate information. Any discrepancies must be reported immediately.

- In the event of any issues arising from this agreement, customers may contact Custom Creations by Jackie  directly for resolution
